  1. Chronic Disease Electronic Management System (CDEMS)
    www.cdems.com
    The CDEMS is a registry program for better management of chronic health conditions developed by the Washington Department of Health with funding by the Centers for Disease Control and Prevention. As public domain software, users may freely make changes to the program. CDEMS is pre-coded to track diabetes and adult preventive health but is customizable to change those tracking measures or define measures for monitoring other chronic conditions. Printed progress notes, patient lists, and summary reports generated from the registry database can alter the way services are delivered and measure quality improvement efforts.

  4. WorldVistA
    www.worldvista.org
    WorldVistA provides open source electronic health record and health information system based on and compatible with the U.S. Department of Veterans Affairs (VA) VistA®. WorldVistA helps those who choose the VistA® system to successfully master, install, and maintain the software for their own use.

    This product meets a set of criteria for functionality (ability to create and manage electronic records for all patients, as well as automating workflow in a physician's office), interoperability (a first step in the ability to receive and send electronic data to other entities such as laboratories), and security (ability to keep patients' information safe).

    Features include:

    • Functionality tailored to meet the needs of clinics and physician offices, such as:

  5. The Medem Network: Connecting Physicians and Patients

    Founded by seven major medical societies, including American Medical Association, American Academy of Pediatrics, and American College of OB/GYN, Medem has established a physician-patient communications network, to facilitate online access to information and care for more than 100,000 physicians, their practices, and their patients. Physicians can communicate with their patients, answer questions, and renew prescriptions. Patients can maintain their own health record and sign up for education programs, including type 2 diabetes, and receive automated health information through secure email.   6. Resource and Patient Management System (RPMS) from the Indian Health Service

    RPMS is an integrated solution for the management of clinical and administrative information in health care facilities of various sizes and orientations. Flexible hardware configurations, over 50 software applications, and network communication components combine to provide a comprehensive clinical, financial, and administrative solution.
